Membership Information
*Our practice does not bill insurance. Your only bill is the monthly membership fee which covers unlimited visits and communication with your doctor. There are no additional co-pays or unexpected charges. Like all primary care clinics, Haden Family Medicine can still refer and order testing for patients who have medical needs outside the scope of this practice.*
Prices: ​
*One time $75/household charged at time of registration.
Children 0-18: $40/month
Age 19-64: $75/month
Age 65+: $110/month
Family Membership: $250/month for up to 5 children in the same household
DOT Physicals: included with Membership
- $150 for nonmembers
